Study Plan/Record is a footprint between student and supervisors throughout the program from the beginning to the completion. It will help review the student's academic work through the years. Submission of Study Plan/Record does not constitute approval of course registration, or leave of absence. For the detail about how to fill and submission time, check the handbook. Update the record when anything newly added or changed.

For students enrolled in or after April 2018 (On-Campus Use Only)
